CATHETER INSERTION AND CARE BUNDLE- Proposed implementation plan with outcome measures.(FOR CAPSTONE PAPER) PICOT to use – Does the implementation of catheter insertion and care bundle in acute care patients with indwelling urinary catheters, reduce catheter-associated urinary tract infections (CAUTIs) rate per 1000 patient days over a six-month period? Example of what a proposed implementation plan with outcome measures looks like. This proposal will be implemented on a 36-bed general medicine unit. The CAUTI rates on this unit are already being tracked on a quarterly basis (see Appendix A). Going back 6 months, the CAUTI rate will be noted. This will be the pre-intervention phase. After implementation of the proposal the CAUTI rates will continue to be tracked. They will be tracked for 6 months after implementation, which will be the post-intervention phase. These will then be compared to one another. While this will be the ultimate outcome measure, this is not the only measures that will need to be traced. The first part of this proposal involves Foley bundle compliance. In order to be sure that this is adhered to, audits will have to be performed each shift. On this unit, there is always a charge nurse each shift. They were given audit sheets (see Appendix B), which is basically a check off form. During shift report the charge nurses are responsible for reporting to one another of who on the unit has a catheter. They will go into these patient’s room with the audit sheet, as well as speak with the covering nurse individually to ensure all of the measures are adhered to. These forms will be returned to the writer, so that bundle compliance can be adequately tracked. The educational aspect of the proposal will involve two different processes. This unit requires a staff huddle to be performed prior to each shift. There are always huddle points provided by management to include in the day to day huddles. This writer has provided huddle points to be included in the huddle each shift in regards to Foley bundles and CAUTI prevention. This is also an opportunity for patients on the unit with catheters to be identified. Having these points discussed prior to the shift beginning will stress the importance of Foley bundles and ensure that it is adhered to during the shift. The second process regarding the educational aspect of the proposal is a workflow reminder (see Appendix C) which was provided for everyone on the unit. Each staff member uses a rolling computer during their shift. A laminated copy of each aspect of the Foley bundle has been attached to each rolling computer on the unit. This will ensure that staff does not miss any components of the bundle, as well as serve as a reminder that these measures must be completed during the shift.
