Part 1 The Integrative Case Study –50 points for final written product
Each student will develop a case report based on a client system from the student’s field practicum experience. Each of the competencies developed in the social work education program will be examined through the lens of this case file. Reflect on your class experiences and earlier assignments in the program to help you complete this report. You should plan on discussing this assignment with your field instructor as soon as possible. The case report should contain, but not necessarily be limited to, the following sections:
Part A: Context for the Case—Identifying information about the field practicum agency/organization. Include the following information:
Who started the agency including when, how, and why?
What is the agency’s purpose/mission statement? How does the mission statement relate to the issues of human rights and social and economic justice?
What specific models or specific philosophy of practice (if any) guide the work of the agency?
Does your agency have a policy and procedures handbook? Describe this handbook.
Does your agency have identification with any faith tradition? If so, how does that affect or inform the practice of the agency? How is faith expressed or implemented in the context of its work? If there is no faith association, how does your agency view and relate to the faith community?
What is the governance structure of the agency? Discuss the Board of Directors and/or Advisory Board and the differences in roles and functions of each as governance mechanisms for the agency’s work.
What is the agency’s policy related to ethical behavior? How does this relate to the NASW Code of Ethics?
Fully discuss funding sources for the agency.
What are the programs and services of the agency?
What are the specific eligibility rules for clients to receive services?
What are the demographics of the clients served?
Describe the agency’s organizational culture as you have experienced it in your field practice.
Part B: Your Selected Client System—
Who is your client? Specify and describe the client system (is this the individual, a family, or an organization?) (If it is an individual, then be sure to describe in detail the person’s family, friends, and any outside agencies involved with them.) Describe the scope of your work with this system, including demographics, culture and ethnographic group, and the client system’s membership in a vulnerable population group. Explain how or why they might be included as a vulnerable group.
Include at least one diagnostic and/or assessment tool that depicts the client system, e.g. psychosocial history, treatment plan, genogram, eco-map, organizational chart, flow chart, community analysis.
