During your field education experience, you will interact with multiple clients. As you interact with clients and review your process recordings, you might discover that one client stands out. This may be due to the services needed or a potential case history that interests you.
As a future social worker, preparing a case presentation allows you to present social work practice skills demonstrated in addressing client needs to your colleagues.
For this Assignment, you will submit a case presentation of a client you encountered during your field education experience
Create a Case Presentation (Power Point w/ speaker notes – 4-5 slides) that includes the following: (Slides should have imagery and few words, not speaker notes)
A description of your client to include demographics, presenting problem, goal, legal/ethical considerations, assessment, and proposed treatment/social services delivery plan and the inclusion of termination plans if applicable
An explanation of whether interacting with your client demonstrated social work practice skills
Identification of potential social work skills not demonstrated in your agency or field placement to include a proposed professional development plan
An explanation of how preparing and engaging in a formal case presentation represents a component of professional social work
Case: Mr. J.
Client: Mr. J is a 68 year old Caucasian Man. He is an Army Veteran and bases the majority of his identity in his time in the Army.
Ethical Dilemma: He suffers from dementia and isn’t of sound mind to make medical decisions. Conflict lies within the immediate family as to the next best steps to care for Mr. J. Additionally Mr. J has moved several times in assisted living on the past few months as he is too much for the staff handle and results in an elopement risk. He refuses to remain compliant with medication regiment which results in violent outburst with staff caring for him.
Assessment: He is lonely and disconnected from family. He lacks the capacity to understand his restrictions physically which resulted in assisted living after his recent stroke. Lacks insight with his diagnosis, remaining isolated from others. He has a history of major depression, anxiety and a recent diagnosis of dementia. He can’t remain alert and lacks ability to connect to current place, people and time. Has negavtive interactions with family when they visit which has resulted in many family members failure to return for visits. While his son and daughter remain supportive to his care and provide financially for him, they are at odds over the best placement for their dad.
Strengths: Mr. J. functions well considering his recent setbacks due to a stroke. He engages well with new people for short periods of time although he lacks the ability to maintain eye contact. He is open to change ie new caregivers and new assisted living environments. He has a strong connection to his Christian faith and Military background.
Referral & Intake: AERS Eval for medical waiver. Functional Eval with yearly redertermination for State Medicare funding.
Use of Role Theory: (please expand upon the following) View how he can accept his new role. Behavior according to this theory. Explore past, versus present and importance of his new role in society.
Intervention for therapy: Use of CBT (regarding dimentia, depression and anxiety)
Intervention – (research and expand) Walks (physical activity for mood). Goal of becoming medically compliant. Visits to Adult Day Center at Bureau of Aging for engagement with peers.
