The Psychopharmacology Case Presentation is a scholarly inquiry into assembling essential elements of a new DNP graduate to initiate psychopharmacology into practice optimally.
The Psychopharmacology Case Presentation will be the Signature Assignment in the School of Nursing and Health Professions evaluation system. A description of the assignment and its grading rubric are provided. This assignment is the Signature Assignment and is due week 7 of the course. The Psychopharmacology Case Presentation provides a student with an opportunity to operationalize and synthesize a blending of therapeutic approaches with psychopharmacology and demonstrate the ability to understand when a certain medication is appropriate and when it is not. There will be two case presentations submitted to Live Text. These presentations will be submitted via Voice over Power Point or a video platform such as YouTube or Vimeo.
The following will be expected of each student: Identify 1 psychotropic medication from the list posted in blackboard (cannot use the same medication that you used for the teaching tool assignment). Develop (1) one case study that shows how, why and when you would use this medication and (1) one that shows how, why and when you would NOT use this medication. You may use real or fabricated patients for these studies. Each case summary will include a full psychiatric initial evaluation of the patient, a summary of their current condition and treatments used previously. You must justify your decision about the use or non-use of the medication with evidence-based research.
Provide information on your chosen medication prior to the case studies. Briefly and succinctly, tell why it is appropriate or not appropriate to use the chosen medication at the conclusion of the case study. Demonstrate how each of the patient’s medications assist in their treatment plan. Provide a discussion of the expected overall encounter with the sample patient including potential barriers/issues that you might anticipate when working with the clients and explaining why or why not you have chosen or ruled out this medication for them. Provide an example of how culture and diversity enter into your decision making process for your patient or possible adaptations that may be required. Provide a summary of conclusions and 3 statements of what you learned from this exercise. The power point/video should be no longer than 10 minutes, submitted in a timely manner and contain 2 case studies. References as necessary.

Information about the Medication
Wellbutrin can be employed to treat mental health conditions like major depressive mental health disorders. In some cases, the drug is used as a supportive intervention for quitting smoking. Bupropion helps patients to elevate their mood or feelings of wellbeing. Additionally, the drug works by creating a balance in some of the neurotransmitters present in a person’s brain. Also, Wellbutrin is not sold over the counter without a signed prescription from a qualified mental health practitioner.
Identification of the Medication
Wellbutrin is the chosen drug that will be focused on this medical case study analysis. The drug is approved by the FDA as an effective antidepressant for treating depression and associated medical conditions. The drug is also sold with other brand names, with the most common brands being Bupropion and Zyban (Fava et al., 2005). The interaction level of the drug with other non-pharmacologic substances is high so a patient can use non-medical regimens to further support his/her treatment process.

Patient X attends hospitals with a range of symptoms that indicate she could be suffering depression. The symptoms manifested by the patient X include anxiety, agitation, and sleeplessness, lack of concentration, weight loss, and poor appetite. Patient X has gone to the hospital and appears calm, relaxed and attentive. Also, the patients are 56 years old, have a normal heart rate and her language skills are intact. However, from physical observation, she appears to be in a depressed mood. Patient has tried committing suicide 4 times in the past and was diagnosed with major depressive disorder in 2014.

Also, patient X has 2 children living in a different state. The children do not visit home often. Patient lives alone because she is a widow and she does not have any friends near home. The patient’s PHI was obtained from the patient’s past medical history entered in the electronic patients’ health system in a different hospital. The PHI database where the information was obtained was updated 2 weeks ago. As such, the reliability of the information is high. Patient complains lack of sleep and also, she has a very poor appetite in the past 4 months.

The vitamins did not boost her appetite. She had been diagnosed with recurrent depression in the past and she suspects she might have depression now. The patient is using piriton to help with sleep but it has not helped. However, she is not using any other medications. The patient had been admitted for attempted suicide 4 times in the past. Patient was treated using anxiolytic drugs in 2014 and 2016 to help with depression.
The patient is addicted to tobacco and she has been smoking for 23 years. Nevertheless, she has no other medication. Also, the patient attained psychiatric treat in 2014 and was treated for depression in 2016. Patient has no recorded allergies in the patient. The patient’s mother diagnosed with bipolar disorder while her daughter was diagnosed and treated for depression. Additionally, the patient’s father committed suicide while her brother is an alcoholic. The patient grew up in a domestically violent homestead and her parents got divorced while she was 11 years old. Also, the patient was bullied in high school and she is currently obese, which has affected her self-esteem and ability to create friends.

In reference to mental status examination, patient X’s assets are the mental health assessment examination, his liabilities are poor friends that are not supportive of the patient’s mental health, while his physical examination findings reveal that the patient has fatigue, experiences regular headaches and has no appetite. Also, the differential diagnosis of Patient X’s condition is anxiety. Additionally, that is because a patient with anxiety also experiences a loss of sleep and appetite. Example of Culture and Diversity in The Decision Making Process and Necessary Adaptations
Culture and diversity factors have a great impact on how medical practitioners make decisions affecting their clients in a healthcare environment. In the featured context, the depressed patients are a 56-year-old female Muslim patient and a 48-year-old female. Due to their religious considerations, they stated that they felt comfortable having her physical examination executed by a female nursing practitioner. Also, the patients only made decisions on their treatment only in the presence of a female Head Nurse at the clinical facility. Nevertheless, since the medical practitioner in charge of treating both patients has prior experience in dealing with clients from diverse cultural backgrounds, there was no need for any necessary adaptation by the medical officers.
