Description/Paper Instructions Prior to therapy sessions start, it is significant that the therapist maintains a good relationship with the client. Establishing the therapeutic relationship can be done through multiple techniques that the therapist should work on. As a therapist, therapeutic relationship is a key for me in order to have a successful treatment session. First, using effective listening skill while the client is telling her story and showing empathy would encourage the client to speak more and feel comfortable with the therapist. Also, being genuine and respectful with the client could make the client feel valuable and understood. In addition to that, showing moderate disclosure is important for building the therapeutic relationship as it makes it easier for the client to be open about her thoughts. As well as, avoiding judgment and regard the client unconditionally. During the interview, I will be using semi-structured technique where I will not be asking Wahiba formalized questions. I will be including more open-ended questions in order to encourage her to discuss her situation more freely rather than answering close-ended questions. However, I am going to prepare a set of questions before the interview where it would make the interview go more smoothly. This interviewing technique will result with more detailed answers from Wahiba and offer more depth to the interview. Also, it enhances the chance of developing more understanding and sense of her problems. Some examples of the prepared questions are:
In order to help Wahiba to notice relationships between thinking and behavior, I will encourage her to differentiate between thoughts and behaviors. Wahiba needs to understand the influence of thinking on her behavior and feelings. This can be done through giving examples of her beliefs such as “she developed a sense of being odd and unacceptable” while the word “sense” is a feeling word. This belief has influenced her behavior where she became shy and her feelings and physiology when she experienced tearfulness, loss of interest, irritation and exhaustion. Wahiba diagnosed with depression disorder, she need help to get rid from it to live her life normally and without problems. First, I am going to read her information through her lifestyle and her own life to have a feedback. After meeting Wahiba I will ask her about herself to get more information to start solving her problem and write a great plan to help her. While writing down her information I am going to write her problem on central CBT technique which is ABC form. ABC form will help me to differentiate between the actual trigger event, and her thoughts, feelings, beliefs and behaviors. A: Her husband died of a heart attack seven years ago B: describes herself as ‘always being extremely shy and anxious C: loss of interest, irritation, exhaustion (emotions) – disrupted sleep, poor appetite (physical sensation) – finding it hard to manage daily tasks such as shopping, and had cut down on her social activities (behavior) . The negative automatic thoughts which Wahiba has which contains “I am selfish”, “I should be sounding post and always put others first”, “ I am always shy and anxious” are very high on self-rating of beliefs, that shows that she is totally believed her negative thoughts. The techniques I’ll use to help Wahiba to avoid and be aware from her negative thoughts is by open up the thoughts and feeling to a broader perspective is by Self-monitoring of thoughts in identifying the negative thoughts and use Socratic questioning to help Wahiba answer her own questions this would help her evaluating her thoughts, behavior and her goals this would improve her life in her coming years. While helping Wahiba identify the impact and consequences her thoughts and behaviors, she is going test her thoughts in reality rather than just talk about it. Also, consequence analysis would help her examine specific behaviors that she wants to change, with this model, she will start by identifying and writing down a specific negative behavior, what led up to it (the antecedent) and what happened right after the event (the consequence). The tools I will employ to help get a better understanding of Wahiba’s mental health is the Beck’s depression inventory, it would help assess her and how severe her depression is from the results that will be shown. The methods I might employ to help Wahiba’s deal with them is by using the cognitive behavioral therapy, to help her understand her thoughts and to avoid distorted and the negative thinking which could make her behave worse than she ever did. CBT sessions are usually 50-16 minutes long. Wahiba needs 8-12 sessions, we need to achieve the changes of her beliefs and thoughts. The sessions would divide to three Modules which is thoughts, activities and relationships. First, we are going to start with thoughts module, how Wahiba’s thought affect her mood. Thought module contains (Sessions 1-4). The first session will go through defining the thoughts of Wahiba. The next three sessions will work with different types of thinking errors and dysfunctional thoughts associated with depression, the design of the third session meets the purpose of providing the participant with strategies for increasing positive thoughts and decreasing unhealthy or dysfunctional negative thoughts and decrease depressive thoughts. The second module its under Activities title (sessions 5-8) how Wahiba’s activities affect her mood. There is a discussion on how the presence of depression can limit Wahiba in pleasant activities, which in turn, increases depressive symptoms. During these 4 sessions, pleasant activities are defined and obstacles for engaging in them are identified. This module also works with how learning to establish clear goals can help decrease depression. The main purpose in activities module is that Wahiba increase her control over her life and learn to have more freedom and choices, me and Wahiba together, goals and activities are established that will help improve her mood. The last module which named by relationships (sessions 9-12) is about how relationships affect mood, Wahiba in this session will learn to identify and strengthen her social support networks. The last sessions integrate themes from the previous modules. I and Wahiba together going to examine how thoughts affect the activities, social support and relationships that Wahiba engages in. Exercises will be used to teach assertive communication skills that will help her establish healthy satisfying relationships. By using this manual guide, it will be useful to help Wahiba because it presents the fundamental areas you show focus on during sessions.
