You will design a Personal Action Plan for the hypothetical client avatar you developed in Week 1 for this project. Outline the exact steps you would take to promote behavior change, taking into account the client’s health risk(s), diagnosis, psychosocial variables, and confidence in his or her ability to change.
Goals: Choose one behavior that your client wants to modify and write out five S.M.A.R.T. goals for behavior change that are specific, measurable, achievable, realistic, and trackable. Describe how these objectives will enhance the client’s emotional and physical health, as well as his or her confidence in achieving his or her objectives. Consider any ethical difficulties that arise throughout the formulation of these objectives.
Barriers: Examine psychological influences on the client’s health, wellness, and therapy. Make a note of any physical, social, moral, cultural, and/or financial barriers that may prevent the client from reaching their objectives.
Barrier-Breaking Strategies:
To assist the client in overcoming change hurdles, use three counseling techniques and three motivational interviewing strategies. A group setting must be used for at least one of the counseling strategies. Compare and contrast three behavioral change theories/models, and explain how each can help the client’s emotional and physical well-being. At least three peer-reviewed articles from the university library should be used to back up your response.
External Support: Discuss how the client’s family, spouse, or caretakers could be included in the action plan. Then, choose three people to be a part of your client’s care team. To begin, give roles and responsibilities to each team member. Specify how you’ll communicate with each other and how often you’ll meet. Explain how you might involve the entire practice team in providing systemic assistance to the client. Look for recommended criteria in the university library and mention at least one peer-reviewed source.
Plan for Follow-Up: Describe how you will maintain healthy practices with the client between visits and after termination. Recommend two social and/or environmental resources for continuous assistance (e.g., peer-led groups, patient education programs). At least one peer-reviewed source from the university library should be used to support your recommendations.
The paper must be seven to eight double-spaced pages long (excluding the title and references pages) and formatted according to the Ashford Writing Center’s APA style guidelines.
