Mr. NB, a 49 years old moderately obese man, came to the emergency department with a month’s history of progressive headache presented to the emergency department with nausea, vomiting, and visual disturbance. Headache was unrelieved by medication. It is noted that he was previously diagnosed with hypertension (HTN) and was prescribed with Beta-blockers but has not taken medicine for a year now for an unspecified reason. He is an active tobacco smoker and drinks alcohol occasionally. Denies of recreational drug use. He has family history of HTN to both father and mother. On arrival, his blood pressure was noted to be 180/110. MRI of the brain demonstrated scattered lesions with increased intensity in the occipital and posterior parietal lobes. But no acute CVA noted. The clinical presentation is suggestive of hypertensive emergency and hypertensive encephalopathy. He has a college-level education. After knowing about his current condition and future possibility of having stroke, the patient is more willing to learn about the ways to control his blood pressure and bring change in lifestyle.

This app can be downloaded from Apple Appstore and google play store for free of cost. You can also login through your computer to the CareClinic website. The app can be used to track symptoms, activity, dietary intake, and medication reminders (Careclinic.io, 2020). It is intended to assist anyone with management and monitoring for hypertension. It helps to help personalize your health monitoring and make it easier to see where you can adjust strategies to enhance your hypertension management.
