Noise in the perioperative setting is of two varieties: noise created by OR personnel and noise generated by equipment. The noise created by personnel includes
- Essential and nonessential conversations,
- Door openings and closings, and
- The number of personnel in the OR.
The noise generated by equipment and devices may originate from several sources, including
- Portable communication devices (eg, pagers, cellular phones, handheld two-way radio transceivers like walkie-talkies)
- Fixed communication devices (eg, overhead paging systems, intercoms, telephones),
- Electronic music devices (eg, radios, CD players, digital audio players), and
- Medical equipment and devices (eg, radiology equipment, waste management systems like vacuum system; smoke evacuators, powered surgical instruments, monitors, clinical and alert alarms, metal instruments).

Effects of Noise inside the OR
Burlingame (2019) mentioned that noise in the OR can interfere with team members who are trying to talk to one another regarding patient care. Several researches measured the effect of intraoperative noise on communication during a percutaneous nephrolitothomy by Cheriyan et al (….. cited in AORN 2019). The researchers used 3 different noise levels: ambient noise (53.49 A-weighted decibels or dBA), equipment-in-use noise (78.79 dBA), and equipment and music noise (81.78 dBA). In the procedure, the surgeon spoke 20 different medical words or phrases five times, and the first assistant, anesthesia professional, and RN circulator wrote down what the surgeon said. The rate of correct responses decreased for all recipients with each increase in the noise level from 97%, 81%, and down to 56%. The researchers concluded that noise pollution impeded effective intraoperative communication.
Interventions to decrease Noise inside the OR
Interventions to decrease noise levels are dependent on the source. According to Burlingame (2019) the use of fixed communication devices should be set only for essential communications at the lowest possible volume and as an alternative to opening the door and entering the OR suite to communicate with OR staff inside the room. It is preferred to be used rather than opening doors because opening doors causes greater distraction and affects airflow in the surgical suite. For electronic music devices, the AORN recommends that the volume level should be low enough to allow communication among team members.
Burlingame (2019) mentioned the interventions for decreasing noise created by personnel include:
- Limiting nonessential conversations,
- Controlling the tone and volume of essential conversations,
- Limiting the number of personnel in the OR, and
- Limiting the number of door openings by using communication devices instead of opening the door for the sole purpose of communicating.
Furthermore, he listed the interventions to control noise generated by portable communication devices which include:
- Placing the device on vibrate or silent mode,
