M.A is a 54-year-old female Hispanic present for family counseling/psychotherapy for mental health disorders management with her two daughters. the daughters reported a decline in mental health want her to get help. The client lost her husband to multiple health problems a two years ago. they were married for 28 years. the client has a fraternal twins sister living in California that she is not close to. client dad died in 2005, her mom is in new York. She is still grieving over him she feels lonely and break down emotionally sometime. she has two daughters and one grandchild and son in law. The client parent she lives by herself in an apartment and refused to stay with one of her daughters for support until she gets better. the daughters are very worry about her but she fight them when they to help her. The client with a past mental diagnosis of Depression, Bipolar disorder, general anxiety, insomnia. she has a medical history, of chronic migraine, hypertension, arthritis, and hip replacement 2 years ago. Client alert and oriented and well groom and nourished. The client reports that she went to urgent care for waist pain and they prescribe her Gabapentin for pain which helps a little bit but not much about a week ago. Medication- She takes Ambien, trazodone, and Seroquel and olanzapine for insomnia, bipolar, anxiety, depression. Client report a suicide attempt by overdosing on her Seroquel which she was admitted for as an inpatient back in February. The client’s mood is anxious and depressive, passive fleeting suicidal ideation thoughts. Client over talk therapist frequently, and mild rambling. Client report smoking THC- marijuana but trying to decrease the amount and eventually quit. Intervention- Cognitive Behavioral Thereby, Dialectical Behavioral Therapy, supportive, and structural family therapy approach to help manage symptoms of hopelessness, self-fulfilling profecey, anxiety, insomnia, and depression, increase quality and quantity of life/sleep. psychoeducation intervention regarding medication compliance, self-care, sleep hygiene, and social interaction, cognitive refocusing, communication skills, exploration of coping skills, Discussion about safety plan, preventative service and to call her someone when she is having suicidal thought. will refer client to higher level of care if symptoms not improve by next sessionAssignment
