Ginger Johnson was 37 years old when she conceived her third pregnancy. Per her doctor’s recommendation, Ginger underwent genetic screening tests, which revealed that the fetus was positive for Down syndrome.
After intense and very emotional discussions with her husband and extended family about the possibility of an abortion, Ginger decided to have the baby. During the pregnancy, Ginger developed gestational diabetes, and baby Jack was born 6 weeks premature by emergency caesarean section.
Assessment
Jack had feeding difficulties due to low muscle tone requiring oral motor therapy every 3 hours at birth followed by once-weekly therapy until age 2. He could not tolerate any dairy or gluten products from birth.
His low muscular tone also required physical and occupational therapy one to three times per week. He had hip dysplasia that required various adaptive measures, and his feet, ankles, and lower calves required orthotic braces. Unlike over 50% of children with Down syndrome, Jack had no cardiac anomalies.
Jack had strabismus from birth, which required eye patching and glasses for far-sighted vision at approximately 18 months. He also had bilateral eustachian tubes and underwent an adenoidectomy at age 3.
Jack was plagued with chronic diarrhea for his first 6 years, which resolved with elimination of intestinal bacteria and use of digestive enzymes.
Jack, age 12, Caucasian male, was born with Down syndrome. Jack uses variety of interprofessional medical services that have yielded success in the day-to-day management of Jack’s health issues.
His growth chart illustrated no growth from ages 2 to 3, and he was diagnosed with a pituitary deficiency. Human growth hormone treatment was initiated at age 3 and will continue through puberty.
Jack was also diagnosed with attention-deficit hyperactivity disorder (ADHD) but did not respond well to stimulant medications at age 5; therefore, a second-line medication was prescribed.
As Jack’s immune system improved, he experienced fewer illnesses, with the exception of contracting pneumonia at age 7. He was hospitalized for 10 days of which 3 days were in the intensive care unit (ICU).
Jack was diagnosed as borderline diabetic, and his physician recommended a Paleo diet to help normalize glucose. Jack began puberty at 10½ and was diagnosed with scoliosis at age 11.
Jack attends a public elementary school that has a special program for children with IDDs, where he has had the same teacher for the last 4 years. But he will be transitioning to middle school this year and be introduced to a new teacher.
Jack’s family lives in a middle-class area of a small city and consists of his mother Ginger, father Sam, and sisters Jill (15) and Jessica (17). Ginger is a stay-at-home mother, and Sam is an accountant for an insurance company.
Health insurance is provided by Sam’s company, but the deductible and copayments have risen dramatically in the last few years. With Jessica starting college next year, money is tight and expected to become more difficult.
Jack participates weekly in hippotherapy and maintains the horse stalls as part of his activities. His only sibling, a sister, will leave for college in a few months.
Jack has struggled with an immune-based response, alopecia.
