As an LTC AIT student your task this week will be to demonstrate the knowledge you have obtained from your first weekly rotations.

Two LTC facilities/settings from Your Week 2 Rotation. (LTC Living Arrangements/Facilities)
Some Examples: SNF, including short-term rehab and long-term care skilled nursing facilities, ALFs, ILFs, CCRCs, Memory Care Facilities, Psychiatric LTC Facilities, Pediatric SNFs, Hospice Facilities, Respite Facilities, Developmentally Disabled Facilities, LTC Mental Health Facilities, or other LTC Facilities – be specific.
Two LTC HCBS and/or Community Living Options from Your Week 3 Rotation. (LTC Community and Home-Based Services)
