Alcohol Use Disorder Diagnostic Criteria
The DSM-5 has recently modified the diagnostic criteria for alcohol consumption disorders. A patient can be diagnosed with an Alcohol Use Disorder in one of three severity levels in the DSM-5: mild, moderate, or severe. The number of presenting symptoms out of eleven total possible symptoms determines the severity level. Two to three symptoms constitute a mild presentation, four to five symptoms constitute a moderate presentation, and six or more symptoms constitute a severe presentation (APA, 2013). The following are the eleven criterion symptoms:
“Alcohol is frequently used in higher quantities or for longer periods of time than anticipated.
There is a constant desire to reduce or regulate alcohol consumption, as well as unsuccessful attempts to do so.
A significant amount of time is spent on activities that are required to obtain, use, or recuperate from the effects of alcohol.
A strong desire or drive to consume alcohol is known as a craving.
Recurrent alcohol abuse leads to a failure to meet major role responsibilities at work, school, or at home.
Despite having continuous or recurrent social or interpersonal issues caused or aggravated by the effects of alcohol, some continue to drink.
Alcohol usage causes people to give up or limit important social, occupational, or leisure interests.
Recurrent alcohol consumption in physically risky conditions.
Despite knowing that they have a persistent or recurrent bodily or psychological problem that is likely to have been caused or exacerbated by alcohol, they continue to drink.
Tolerance is described as the need for significantly higher doses of alcohol to achieve drunkenness or the desired effect, or b) a significantly lessened effect with continuing usage of the same amount of alcohol.

Treatment for alcoholism is very dependant on the individual and how devoted they are to the process. Detoxification is the initial stage in treatment for each person. Depending on the patient’s demands and level of dependence, detoxing can be done with or without the use of drugs. Schick Shadel and the use of a CIWA in the inpatient setting are two medically aided models.
Depending on the patient’s determination to stay dedicated to sobriety, medical treatment can be continued with one of two main drugs once the person has detoxed. If the patient drinks alcohol, one of the medications, Antabuse, will cause a negative reaction (Stahls, 2014). The other, Naltrexone, will assist to reduce alcohol cravings (Stahls, 2014). Psychotherapy can also assist the individual in addressing the underlying issues that lead to drinking (Sadock, 2013). Individual and group therapy can be useful in conjunction with CBT to help identify triggers and develop new coping mechanisms (Sadock, 2013). Finally, a group setting, such as Alcoholics Anonymous, can provide continual support and encouragement for sobriety.
Characteristics of the clinical presentation
A patient I worked with was a middle-aged man who had been fired from his job and was going through a divorce. He was fired from his job due to poor attendance, poor quality of work, and showing up to work while inebriated. His marriage was displaying signs of alcoholism because he was drinking more frequently, putting a burden on his finances and becoming easily upset, followed by a blackout. Because of the increased use, the influence on his personal and work life, and the increased urges for use, a comparison of his symptoms with the DSM would reveal that he was suffering from severe alcohol abuse.
