Healthcare is Political Minicase:
Carl has been the Administrator of the “Physicians Imaging Center” in a large and competitive metro area for three years. The Center is owned by 6 Radiologists, all six of which constitute its Governing Board. The metro area has 5 additional larger free-standing imaging centers and 3 hospitals (at which all 6 of his physicians have privileges) that all provide similar services: the market is extremely competitive. The Center has been successful and has been earning thin but consistent profits. Carl’s physicians are considering the purchase of the newest and very expensive medical scanning machine that they believe will make them more competitive.
Carl has run the numbers and has found that with maximum usage the new scanner would be able to pay for itself in about 3 years. However he doesn’t believe that the Center will be able to achieve this level of capacity. In addition, his reading of the literature suggests that the added medical benefit of the new technology is negligible at best. He believes that the Center would gain more competitive edge by using the money for things that directly impact patients such as remodeling the waiting room and updating the electronic phone and scheduling system. Still his physicians are adamant that they need this scanner to remain competitive.
Babett is the CEO of St. Lucy’s, a hospital in the same large metro area. Five years ago the only facilities offering imaging services in this area were the 3 hospitals. Now, in addition to the hospitals, there are 6 physician-owned free standing imaging centers, all of which are strong competitors, all of which are owned by physicians who have staff privileges at St. Lucy’s. Dr. Winkel, the head of St. Lucy’s radiology department is on St. Lucy’s Board of Directors and is not affiliated with any of the free-standing imaging centers. However, she has had an open invitation to join three of the centers and she just today had lunch with Carl, the Administrator of Physicians Imaging Center.
Dr. Winkel has been avidly interested in St Lucy’s purchasing the same new, expensive scanning technology that the Physician’s Imaging Center is considering and had been planning to come to St. Lucy’s Board to request its purchase. But after her lunch with Carl, she is now having second thoughts. Babett, St. Lucy’s CEO, has been hesitant to purchase the new technology because there are so many other far more pressing and important demands on the hospital’s resources, many of which have a direct impact on the quality of care at St. Lucy’s.
Radiology is one of St Lucy’s most profitable areas, due to a great part, because of Dr. Winkel’s excellent reputation, vision, clinical, and management expertise. St. Lucy really cannot afford to lose Dr. Winkel. The very active hospital rumor mill has just informed Babett that Dr. Winkel is being wooed, and likely will soon be stolen, by the Physicians Imaging Center.
