I feel that the Health and Social Services Recovery Support Function is especially helpful as a public health nurse because it deals with just that, the health of the public after a disaster. This area focuses on recovering public health, health care services, behavioral health, behavioral health, long term health issues and more after a disaster (FEMA, 2018). This function was in response to Hurricane Irma, but I believe that it can be a model for all types of disasters in order to restore health to a community after a disaster. It allowed for more training and preparation in response to disasters to better the community during a tragic event and also highlighted areas that needed growth. Reference: FEMA (2018). Many FEMA trailers were given to the members of the community temporarily, while others became permanent owners of them. It took a very long time to get the actual trailers, but nonetheless they did get them. They aren’t the nicest thing to look at, but when you have no where else to go it’s a good resource for some. 3 This last shift at national adult day care was another great experience. I look forward to my last shift at the end of the week. With the Department of Aging arriving sometime within the next few weeks, staff is preparing the facility. This week I worked directly with the program director and assisted him, the physical therapist, and social worker in putting the hard copy charts together of every participant. This was a very lengthy process as it had not been done in over six months. I feel it is important to have these charts together at all times, not just because state reviewers are coming. Health (2016) stated, “Clear and concise medical record documentation is critical to providing patients with quality care, ensuring accurate and timely payment for the services furnished, mitigating malpractice risks, and helping healthcare providers evaluate and plan the patient’s treatment and maintain the continuum of care”. Next, I continued to learn how to complete the quarterly assessments on the participants. Overall, it was another great shift. Reference SCP Health. Some of the factors depicted in the film that inhibited the war on AIDS were funding, political climate, and public support. I think that once HIV/AIDS had come to the country there was some time before it was even determined that there was an epidemic. During this time, people had already been spreading the disease. I do not think that HIV could have been stopped once it already came here, however, I think that it could have been significantly reduced and/or isolated. The filmed shows advocacy within members of the CDC and public health offices. A specific example of advocacy in the movie is when the CDC meets with members of the blood bank committee. The CDC was advocating for testing of all blood to detect the virus. The blood bank officials and members of the FDA opposed this due to the amount of money that would be needed to fund this. According to Edelman, Kudzma, and Mandle (2014), approximately 34 million people are living with HIV/AIDS throughout the world. The highest prevalence rates are seen in low-income countries versus high-income countries. The rate in the United States is 0.6% compared to 25.9% in Swaziland (Edelman et al., 2014). Publicity regarding HIV/AIDS is nowhere near the amount that it was when it was first discovered in the 1980s. According to Park (2016), when HIV/AIDS was first being discovered the only approved drug to treat it was AZT, but now there are dozens of medications to manage the disease. Also, as a society, we have become more educated regarding HIV/AIDS and ways to prevent the spread, including testing and condom use. Edelman, C., Kudzma, E., & Mandle, C. (2014). And the band played on [Motion Picture]. United States: HBO Pictures. 5 According to CDC (2019), since the first case of HIV was reported to the CDC the agency has worked on prevention measures to help stop the spread of HIV/AIDS. The CDC utilizes high-impact prevention to reduce new infections by using scientific-based and cost-effective interventions directed at the populations that are are most vulnerable. These are in areas where HIV prevalence is the highest. Centers for Disease Control and Prevention. (2019). CDC Responds to HIV/AIDS. Women make up a significant portion of the workforce. Both men and women are also working jobs that do not end at 5:00 pm. This may create problems at home, as adults bring home their work assignments and problems home with them. There is also an increased amount of job travel. This causes the other partner who is at home to have increased responsibilities while the other is away at work. When family members come home tired or frustrated this may cause emotional strain at home. Culture also influences psychosocial stressors as well. Individuals who are poor, defined as annual income less than $20,000 a year for a family of four, have less access to medical care (Edelman, Kudzma, & Mandle, 2014). People who are unemployed also reported poor psychological and psychosocial health. According to Edelman et al. (2014), blacks are more likely to be uninsured than non-Hispanic whites. There is also an increased prevalence of heart disease amongst Black Americans. Hispanics are the largest ethnic minority in the country. They have the highest uninsured rate amongst the racial or ethnic groups in the country. Cancer is the second leading cause of death amongst middle-aged Hispanics. As an RN we are exposed to multiple daily stressors on the job. I work 12-hour shifts in the emergency department during peak hours between *** in the morning. Staffing shortages, budget cuts, and increased job demand place multiple psychosocial stressors on me personally. Working long hours away from home and dealing with the stress of working with sick patients who need us can directly affect our personal lives. It is important that as nurses we ensure that we maintain balance in our lives so that we do not succumb to the stressors. Edelman, C., Kudzma, E., & Mandle, C. (2014). Health promotion throughout the life span (8th ed.). St. Louis, MI: Elsevier. 7 It has been reported that 12-h shifts lead to poor performance due to physiological strain, fatigue, burn-out and job dissatisfaction, which consequently negatively impacts patient care and safety. A number of studies of US hospital nurses demonstrated that the risks of making an error are significantly increased when work shifts are longer than 12?h, when nurses work overtime, or when they worked more than 40?h per week. In a number of similar studies, nurses working shifts of 12-h or more and those working overtime, reported lower quality and safety, increased risk of errors and decreased nurses’ vigilance. In contrast, the 12-h shift has been reported to be favorable by nursing staff as they allow greater flexibility for personal responsibilities outside of the workplace.1–3 In a number of qualitative studies, 12-h shift patterns were seen as positive, contributed to staff satisfaction and also both a good recruitment and retention strategy.
