Past medical history: Jonathan reports a history of anxiety that first emerged in high school and recalls worrying needlessly about being late with assignments, disappointing his parents, and losing important friendships. He has no history of treatment for anxiety or other psychiatric disorders.
Family history: Jonathan’s father has untreated anxiety. His mother has hypertension and diabetes mellitus Type 2.
Social history: Jonathan lives with a roommate; they have a good relationship. He has a group of friends and usually sees them on weekends to watch sports.
