Community Funding Sources Activity
You are an OTA working in a community-based setting (community adult day center). Most of your patients are elderly, low income, and attend your center on a daily basis because they cannot stay at home during the day alone. Your organization is contracted by the county to provide limited OT services, and you work as part of an interdisciplinary team which provides social services, nursing, dietary, and physical therapy.
One of your patients, Ben, has been informed that due to a slight increase in his monthly fixed income he is no longer considered ‘low income’ and therefore is not eligible for county funding to cover his attendance. Ben is 61 years-old, is eligible for and receives Medicaid, and has no private insurance or assets. Ben is not a veteran, nor has he ever served in the armed forces. You have worked with Ben for over a year and have witnessed the positive change in him since he began at the center. Ben suffers from the following conditions: CAD, COPD, NIDDM, L LE BKA, and mild confusion. You have provided services which focused on increasing activity tolerance/endurance, balance, as well as included him in groups that focused on money management, crafts, and healthy meal prep. Ben, despite a 1% increase in his monthly income, cannot afford to pay for the adult day center’s services out of pocket and is heartbroken about having to leave. You are aware that if Ben discontinues attendance, he will likely have a decline rather quickly. Please complete the outline below to help Ben:
Part 1: The Role of the OTA on the Interdisciplinary Team
- Describe the OTA’s role on the interdisciplinary team for this patient
- Describe at least one way that the OTA might work collaboratively with each member of the team, and one way that working together may pose as a challenge:
- Physical Therapy
- Social Services
- Nursing
- Dietician
- With regards to the pending discharge, what would be OT’s recommendation to the team for this patient?
- Describe at least one activity/intervention that you could utilize as an OTA which would enhance the services provided by each team member and strengthen the collaborative relationship:
- Physical Therapy
- Social Services
- Nursing
- Dietician
Part 2: The Availability of Alternate Funding Sources
- Identify at least 3 possible sources of alternate funding in your local community which may be able to benefit this patient’s continuance in your program. Remember that you are a therapy provider, and thus listed organizations must specifically cover therapy services (and not solely general health and well-being):
Organization Name Phone Number 1. 2. 3.
- Contact each organization (by phone or in person) listed and present the following questions to them. Record your findings in the chart provided below:
Question 1: Do you provide funding to support community-based therapy services for low-income patients? If not, could you recommend a source?
Question 2: What is your criteria for application/qualifying for aid to assist with therapy services? Are their income/age limitations? How much aid is available (is there a max or cap amount)?
Question 3: How is your program funded? Federal? State? Local funding?
Question 4: Is there a waiting list for services/funding for therapy services through your organization? If so, how long is it?

- If yes, how many sources (out of the 3) seemed like a good option for Ben to assist with
continuance of therapy services?
- If no, were there many alternatives listed or provided which may assist with therapy
services?
- Were you surprised at either the abundance of alternate therapy funding sources available, or the lack thereof? Explain your response.
